6 Utica, NY Utica, NY Full-time Full-time $25 - $27 an hour $25 - $27 an hour The Member Relations Coordinator is responsible for supporting the Director of Member Relations in efforts to ensure that LIFEPlan CCO is providing the highest level of support to its members.
The coordinator will be a part of the Member Relations team, all of whom play an important role in helping to raise awareness, promote Care Coordination services, problem solve issues and provide information and resources to our members.
The coordinator will understand the needs of members and families, providing educational programming, and promoting activities that support the full LIFEPlan membership.
This is a telework opportunity in New York state, or a contiguous state (i.
e.
, Connecticut, Pennsylvania, and Vermont)
Essential Functions:
Participate in Member and Family Advisory Council meetings ensuring members and family voices are fully integrated in, and member participation is maximized and solicited to provide feedback and areas for improvement.
Facilitate Member Representative Committee meetings, manage requests, ensure thorough vetting and criteria is adhered to.
Ensure timely life plan approvals for members are provided if representation is unavailable.
Respond to requests for Member Relations assistance as assigned, in a timely fashion to resolve; support (including peer support), researching/locating resources, facilitating resolution to Life Plan disputes, problem-solving member complaints (last resort), and participation in agency workgroups/committees.
Maximize all possible avenues for resolution.
Participate and facilitate Member Relations events and activities, surveys, forums, drop-in sessions, round table discussions, blogs, social media, etc.
Participate in the development of initiatives that focus on bringing a unique set of services and opportunities that enhance a member's experience, build capacity to support independence, and decrease dependability on system, i.
e.
, Supported Decision Making, Supported Employment, financial planning, employment opportunities at CCO, higher learning/education (SUNY, CUNY), etc.
Participate in partnership events that create meaning and value and provides opportunities for members to participate in community activities/events, i.
e.
, Special Olympics.
Contribute content in collaboration with the Member Relations and Marketing team, to reach, interact and educate diverse membership, language translation services, E News, Knowledge Center, CRT (Community Resource Tool), mailings, website, social media, blogs, explainer videos, soliciting member stories, etc.
Participate in agency departmental meetings as needed to offer input ensuring members voice is incorporated into care management practices, i.
e.
, Learning & Development, content development, etc.
Be an available resource for members, parents and family members and a guide to support and services.
Attend community events at which LIFEPlan CCO can make a positive impact in the community.
Occasional work will be required for special events on evenings or weekends.
Serve as a company representative as assigned.
Provide all required reporting in a timely manner.
Other duties as assigned.
Key Duties & Responsibilities for all Leadership/Management Positions:
Ensures compliance with all pertinent government regulations, and LIFEPlan CCO's policies, procedures, and operating standards.
Implements a continuous quality improvement process for department services and administrative functions.
Assists in the development, implementation and administration of Human Resources policies and programs including compliance with Equal Employment Opportunities and Affirmative Action Programs.
Bachelor's degree in a Human Services or related field.
3 years of similar or related salaried experience in a health home, OPWDD or MSC, or comparable setting Or master's degree
one (1) year as above Or associate degree
four (4) years as above Or HS Diploma/GED
five (5) years as above Experience with coordination between various groups of people, either as part of above or other experience.

Estimated Salary: $20 to $28 per hour based on qualifications.
