Mentor - Day Services GENERAL STATEMENT OF DUTIES:
To assist in the implementation of the person we supports Lifeplan and Staff Action Plan by providing direct care, training and supervision to the extent required by utilizing as many community based resources and supports as appropriate to the plan.

The schedule for this position would be Monday-Friday; no weekends or holidays! ESSENTIAL JOB FUNCTIONS:
The following is a summary of the essential functions of this job.
The incumbent may perform other duties, both major and minor, that are not mentioned below, and essential functions may change from time to time.
Ensure protection from abuse, neglect, mistreatment and exploitation in regard to the health, safety, comfort, and well being for the people we support by adhering to all related policies.
To act as a role model for persons we support, actively demonstrating the principles of respect, dignity, personal rights and age appropriateness.
Provide the extent of assistance necessary with activities of daily living, i.
e.
toileting, eating, ambulation, including the provision of lifting assistance as needed.
Assist in providing meaningful experiences within the community for the people we support.
Provide transportation for outings within the community, following established policies and procedures regarding the use of agency vehicles.
Demonstrate appropriate training skills and understanding of active treatment, age appropriateness and functional programming, both one-to-one and in group activities.
Record and maintain progress reports on the implementation of the Staff Action Plan.
Recognize and reports positive/negative responses to activities, as well as physical symptoms and other concerns to the appropriate team member promptly.
Demonstrates active understanding of behavior principles and positive approaches to behavior.
May on occasion be required to provide personal intervention (physical).
ACCEPTABLE EXPERIENCE AND TRAINING:
Employees will be required to have full range of body motion.
They may also be required to lift and carry items up to 50 pounds, as well as the ability to lift and transfer individuals.
Must be able to transfer and position participants and manipulate equipment such as wheelchairs and special lifts.
Must be able to be trained in emergency first aid, including C.
P.
R.
, medication administration and behavioral interventions as based on site needs.
Corrected vision and hearing to the normal range is necessary.
EDUCATION AND
Experience:
High School diploma or GED.
Requirement of HS diploma or GED may be waived in lieu of NYS Dept of Health licensure as a Certified Nurses Aide (CNA), or comparable training or experience that correlates with the functions of this position.
As a mentor, you will be involved in providing transportation to people we support.
An approved copy of a Drivers Abstract from the Dept of Motor Vehicle must be available.
This requirement may be waived by the department head.

The check will bounce and the victim is left responsible.
