Summary:
Per Diem Position Provides social work services to residents and families.
The social worker assists the physician and other team members in understanding the significant social and emotional factors related to the health problems of the patient.
The social worker participates in the development of the plan of care, prepares clinicals and progress notes, works with family, uses appropriate community resources, participates in discharge planning.
Qualifications:
Master's or Doctoral degree from a school of social work accredited by the council on Social Work Education.
Home Care experience desirable New York State Drivers License or access to transportation.
